About Vitality Support Care

Vitality Support Care Pty Ltd is a registered NDIS provider (ID 4050148821) headquartered in Cranbourne North, Victoria. Founded and directed by Hewad Zamari, the organisation delivers supported accommodation, personal care, community nursing and aged care services across Melbourne and regional Victoria.

Our story

Built in Melbourne's south-east, for the whole of Victoria

Vitality Support Care began with a simple observation: too many people were being fitted to services, instead of services being fitted to people. Participants from multicultural communities were explaining themselves through interpreters to workers who didn't share their language. People leaving hospital were waiting weeks for a suitable home. Families were burning out without reliable respite.

We built Vitality to close those gaps — a provider registered and accountable to the NDIS Quality and Safeguards Commission, staffed by people drawn from the communities we serve, and structured so that accommodation, daily support and clinical care work as one team rather than three phone numbers.

Director Hewad Zamari leads Vitality Support Care hands-on. Participants, families and Support Coordinators deal with decision-makers, not call centres — and that shows in how quickly referrals move from enquiry to service agreement.

Registration & accountability

Registered across twelve NDIS support classes

Our registration (ID 4050148821) is in force until 17 June 2027 and covers high-intensity daily personal activities including tracheostomy management, ventilator management, enteral feeding, complex bowel care, urinary catheter management, subcutaneous injections, complex wound management and severe dysphagia management — delivered under clinical governance by registered nurses.

We also implement Positive Behaviour Support plans, including supports involving restrictive practices, developed by registered behaviour support practitioners.

Our partnership with Trilogy Care

For older Victorians, we deliver in-home aged care under a co-managed Home Care Package model: Trilogy Care is the government-approved provider managing package funds and compliance, while Vitality's team delivers the hands-on care.
