We strictly collect, manage, store, and disclose personal and sensitive health data in accordance with the Privacy Act 1988 and NDIS Quality and Safeguards Commission standards.

1. Purpose and Scope

This policy outlines how Vitality Support Care collects, stores, uses, discloses, and protects your personal and sensitive information. It applies to all participants, their families, legal representatives, staff, and contractors across all operations in Victoria.

2. Types of Information Collected

To provide safe, high-quality disability support and accommodation services (including SIL, MTA, and STA), we are legally required to collect:

  • Personal Information: Name, date of birth, contact details, and emergency contact data.
  • Sensitive & Health Information: NDIS plans, goals, assessments, diagnoses, medical histories, medication charts, behaviour support plans, and allied health reports.
  • Financial Data: NDIS participant numbers, billing details, and plan management contacts.

3. How We Collect Data

We collect information directly from you, your family, or your authorised representatives through digital referral forms, telephone assessments, and face-to-face service agreements.

4. Use and Disclosure of Information

Your information is used to deliver personalised care, verify NDIS eligibility, process payments, and meet compliance requirements. Information is only disclosed beyond this when:

  • You provide explicit, documented consent.
  • Disclosure is necessary to prevent a serious threat to life, health, or public safety.
  • Disclosure is required or authorised by law, court order, NDIS Commission, or ASIC.

5. Data Security and Retention

We utilise secure, password-protected cloud databases and encrypted storage systems to protect records from unauthorised access, loss, or misuse.

Participant records are securely retained for a minimum of seven (7) years from the date of the last service entry before being permanently destroyed.
