Support for Complex Needs
Vitality Support Care supports NDIS participants with complex needs across Melbourne and Victoria by combining supported accommodation, high-intensity daily personal activities, registered nursing and Positive Behaviour Support plan implementation within one registered, clinically governed team.
Complexity needs one accountable team, not five providers
When a participant has intersecting medical, behavioural and housing needs, the risk lives in the gaps between providers. We close the gaps by being one organisation across the whole picture: the home (SIL/MTA), the daily support, the clinical care and the behaviour plan implementation — one roster, one clinical governance framework, one phone number.
What "complex" looks like for us
Participants we routinely support include people with acquired brain injury, psychosocial disability with behaviours of concern, degenerative neurological conditions, ventilation and tracheostomy needs, enteral feeding, and dual disability–health presentations leaving extended hospital stays.
How we keep it safe
- Per-participant clinical care plans
- Competency-assessed workers
- RN supervision and audits
- PBS plan implementation with Commission-compliant reporting
- Escalation pathways agreed with the treating team
- Honest capacity conversations before we say yes — because the wrong placement helps nobody
Complex care questions
Do you accept participants other providers have declined?
We assess honestly — if we can support the person safely, we say yes and design accordingly; if we can't, we say so and help find who can.
Can complex participants live in shared SIL?
Often yes, with the right ratios and housemate matching; sometimes 1:1 arrangements suit better. The assessment decides.
Bring us the whole picture.
We'll tell you honestly what safe support looks like.
