Is Vitality Support Care a registered NDIS provider?

Yes — registration ID 4050148821, approved across twelve registration groups until 17 June 2027, regulated by the NDIS Quality and Safeguards Commission.

Who owns and runs Vitality?

Vitality Support Care Pty Ltd is founded and directed by Hewad Zamari, who leads the organisation hands-on from our Cranbourne North head office. Participants, families and coordinators deal directly with decision-makers.

What areas do you cover?

We support participants across Melbourne — especially the south-east corridor from Cranbourne to Pakenham and Frankston — plus Ballarat, Warragul and Gippsland, the Mornington Peninsula, and Melbourne's north and west including Epping, Wollert and Werribee.

What languages does your team speak?

English, Dari, Pashto, Hazaragi, Arabic, Vietnamese and Mandarin, with gender-matched workers available on request.

Are your workers screened?

All workers hold NDIS Worker Screening and relevant checks, with ongoing training and supervision.

Do I need SDA funding to live in your homes?

No. Our homes are accessible mainstream properties. SIL, MTA or STA funding in your plan is all you need.

Can I keep my existing support workers if I move to you?

In most cases yes — we offer to onboard existing workers during transition so your continuity of care is protected.

How does rent work in SIL?

Rent and board are separate from SIL funding and are agreed transparently before move-in.

What's the difference between SIL, MTA and STA?

SIL = ongoing support where you live. MTA = transitional housing typically up to 90 days. STA = short stays and respite, commonly up to 28 days per year.

Can I trial a home before committing?

Always. We arrange tours and housemate meet-and-greets, and where appropriate a short trial stay using STA funding.

Do you take NDIA-managed, plan-managed and self-managed participants?

Yes — all three. We invoice in line with the current NDIS Pricing Arrangements and Price Limits.

How does pricing work?

Service agreements are aligned to the current NDIS Pricing Arrangements and Price Limits, quoted transparently before any support starts.

How fast can services start?

Standard services usually begin within a week of the service agreement. Hospital discharge placements are assessed within 24–48 hours.

What reporting do coordinators receive?

Scheduled progress reports plus immediate incident notification; format suitable for plan reviews.

What high-intensity supports are you registered for?

Eight certified types: complex bowel care, enteral feeding, tracheostomy management, urinary catheter management, ventilator management, subcutaneous injections, complex wound management and severe dysphagia management — under registered nurse oversight.

Do you provide behaviour support?

We implement Positive Behaviour Support plans developed by registered behaviour support practitioners, including supports involving restrictive practices, in line with our NDIS registration.

Can you support hospital discharges?

Yes — referrals acknowledged same business day, intake assessment within 24–48 hours, with furnished MTA rooms and in-house nursing for complex clinical needs.

How does the Trilogy Care partnership work?

Trilogy Care is the government-approved provider managing your Home Care Package funds and compliance; Vitality Support Care delivers the hands-on care under a co-managed arrangement.

What are the Home Care Package levels?

Level 1 — basic care needs; Level 2 — low-level care needs; Level 3 — intermediate care needs; Level 4 — high-level care needs. It starts with a My Aged Care assessment — call us and we will explain the process.

Can one family use you for both NDIS and aged care?

Yes — many families work with us across both systems: one provider relationship, two funding streams.
